Per State Law, a Physical Exam is mandatory for students entering school for the first time in Kindergarten, (or 1st Grade if student skipped Kindergarten), and for students in grades 6 & 11. The exam may be completed by your family doctor, or by the school doctor (free of charge). If your child had an exam since July 1 of the year prior, you do not need to repeat the exam, just have your doctor complete the form based on the last exam date. As detailed in the parent handbook, failure to submit a completed private physical examination form by the date that the school doctor is scheduled is implied consent for a school examination.
