Each year we ask that parents/guardians update their child's Emergency Care Form so that we have up-to-date health information for your child.
If you have more than one child in the district, you will need to fill out a separate card for each child.
- Please log in to the Sapphire Parent Community Portal.
- Select Student Data Forms under My Backpack.
- Under Forms Awaiting Completion, select Emergency Care Form to complete and submit your ECF for the new school year.
Please note that you must be logged into the Sapphire Parent Community Portal with a parent account and NOT
a student account. If you do not have a parent account and need to create one, please review this document
that walks you through how to create a parent account.
