Physicals and Registration Process
Beginning in 2014, all PIAA CIPPE physical forms are required to be completed online with the exception of Section 6 and Section 7 (see directions below). Please follow this link to access our district's Registration Site to register your child.
Please note: You must register your child for each sport they wish to participate in. For instance, if they participate in cross country in the fall, you will register them for cross country in the fall. If they then wish to participate in wrestling in the winter, you will need to revisit the registration site and register them for wrestling.
In addition to registering for the second sport they wish to participate in, they will also need to turn in
a Section 8 - Recertification Form. Please see the information below under Physicals.
A physical can be obtained free-of-charge by the school district physician or through your own personal doctor. Please see our School District Physical Schedule page for details on when physicals will be conducted by
the district physician.
- Parent completes Section 6 - Health History Form (link below). Please be sure both the parent and student sign and date this form.
- Print out and take Section 7 - Medical Examiner Form to your doctor (link below). The doctor will complete this form after conducing your child's physical.
- Return Section 6 and Section 7 to the Oblock Junior High School Office.
Section 8 - Recertification Form
Please note: Only one physical is required each school year. If your child has already received one physical and participated in one sport, you only need to complete a Section 8 - Recertification Form (link below) and register them for the next sport they wish to participate in.
