Fall Athletic Physicals
Fall athletic physicals are due prior to the start of practice and the health history (section 5) Dr. portion (section 6) must be completed on the PIAA CIPPE forms. Sections 1 - 4 must be completed online by accessing the Athletic Physicals link.
