New Lead Testing Requirement for Students entering Kindergarten in the 18-19 School Year:

The Allegheny County Health Department (ACHD) recently imposed regulations requiring schools to collect proof that students entering Kindergarten have been tested for lead exposure. Lead is a neurotoxin that can have irreversible effects on your child’s health, IQ & ability to learn.  Your child could encounter exposure from many different sources, including the public water supply, as happened in Flint, Michigan. Please remember to ask your Health Care Provider to include the dates of lead testing when completing school forms. If your Kindergarten age child has not previously had lead testing, it is recommended that he or she get tested as soon as possible. The ACHD provides this test to un- or underinsured families. Please refer to the brochure in the Registration Packet, or you may find it posted on the Nursing Services section of the district’s website. For any concerns or questions, please contact your school nurse.

NURSING SERVICES
Plum's Commitment to You & Your Child
The Plum Borough School District employs 3 Certified School Nurses who split time between their assigned buildings. Because the Plum Borough School District makes your child's health and well-being  a top priority, when the Certified School Nurse is not in the building, there is an RN or LPN working in the Health Room to take care of your child. If your child has special health needs, we encourage you to contact your Certified School Nurse to plan for your child's care even before starting school.
Laws and Policies
Our School Nursing Services are governed by State Laws and School District Policy. Guidelines and Regulations are imposed by the Pennsylvania Department of Health, the Pennsylvania Department of Education, and the Allegheny County Department of Health. In accordance with the law, all students receive an annual Height, Weight, Body Mass Index, and Vision Screening. Hearing Screening is completed for students in grades K-3, 7 & 11. Scoliosis screening is required for students in grades 6 & 7.
Emergency Contact Information
A new Emergency Care Form must be submitted at the start of each new school year. It is very important to keep this information up to date throughout the entire school year in case the nurse should need to contact you if your child is ill or injured. We also ask for additional contacts such as neighbors or relatives that we could contact if you are unable to be reached. In addition, for the safety of your child we will only release your child to a person listed on this form.
Immunization Information
Immunization Information must be Provided to the school nurse. By law, your child must have at least one dose of each required vaccine before he / she will be permitted to enter school. PA LAW REQUIRES THAT WITHIN 5 DAYS OF STARTING SCHOOL, the Nurse's Office be provided with proof that your child has received all "Required Vaccines." If further doses of missing  vaccine(s) are not medically appropriate during the first 5 days of school, then on or beore the 5th day, you must submit a statement, signed by your health care provider, listing the date(s) the remaining dose(s) will be given. The Allegheny County Health Department provides vaccination services. You may call them @ 412-578-8062 for clinic location and hours. Contact the nurse's office for information on obtaining a Medical or Religious / Moral Exemption.
Health History
A Health History form should be completed for all students entering Plum Borough School District for the first time. If your child has a specific health condition such as Severe Allergy, Asthma, Diabetes, or Epilepsy, please also complete the corresponding additional information form and submit to the nurse. Forms can be downloaded from the right side of the web page, or obtained from the School Nurse.
Medication Regulations
According to state law, medication cannot be administered to any student unless the nurse is provided with a signed order from a licensed prescriber. An order is required for all prescription medications, over the counter medications, and nutritional or herbal supplements. Any medication sent to school must be in the original manufacturer’s packaging or a properly labeled pharmacy medication bottle.  School policy also requires signed parent permission. The pharmacy label on the bottle does not fulfill the requirement for a signed order
Physical Exam
A Physical Exam is mandatory, by state law, for students entering school for the first time in Kindergarten, (or 1st Grade if student skipped Kindergarten), and for students in grades 6 & 11. The exam may be completed by your family doctor, or by the school doctor. If your child had an exam within 1 year of the beginning of the grade in which the exam is due, you do not need to repeat the exam, just have your physician complete the form based upon the last exam.  According to the guidelines detailed in the parent handbook, failure to submit a completed form by January 15th, implies consent for a school exam to be performed.
Dental Exam
A Dental Exam is also mandatory, by state law, for students entering school for the first time in Kindergarten (or 1st Grade if student skipped Kindergarten), and for students in grades 3 & 7. Your family dentist may complete the exam, or the school dentist may do a dental screening. If your child had an exam within 1 year of the beginning of the grade in which the exam is due, you do not need to repeat the exam, just have your dentist complete the form based upon the last exam. According to the guidelines detailed in the parent handbook, failure to submit a completed form by January 15th, implies consent for a school exam to be performed.
Contagious Diseases
Please assist us in decreasing the spreading of contagious diseases! Notify your school nurse if your child has an illness such as strep throat, chicken pox, pertussis, flu, etc. If your child is ill with a fever, keep him/ her home until they are fever free for 24 hours without having taken Tylenol or Ibuprofen. Also during cold & flu season, remember to teach proper hand washing, coughing into fabric covering the elbow or shoulder, and encourage the use of tissues and hand sanitizer.
Announcements & News

If the form is not received by this date, a school exam will be scheduled. Thank you for your cooperation.
